The Licensed Practical Nurse (LPN) provides direct nursing care to residents at the Southern Utah Veterans Home. The LPN works under the supervision of a Registered Nurse (RN) and collaborates with the healthcare team to ensure high-quality, compassionate care for our veteran residents.
- Administer medications and treatments as prescribed by physicians and according to facility policies.
- Monitor and document residents’ health status, including vital signs, symptoms, and changes in condition.
- Assist with the development and implementation of individualized care plans.
- Provide wound care, catheter care, and other nursing procedures as required.
- Collaborate with RNs, physicians, and other healthcare professionals to coordinate resident care.
- Maintain accurate and timely documentation in residents’ medical records.
- Respond promptly to resident needs and emergencies.
- Educate residents and their families on health maintenance and disease prevention.
- Adhere to all facility policies, procedures, and regulatory requirements.
- Current and valid Licensed Practical Nurse (LPN) license in the state of Utah.
- Minimum of 1 year nursing experience preferred, preferably in long-term care or geriatric settings.
- Basic Life Support (BLS) certification required.
- Strong communication and interpersonal skills.
- Ability to work effectively both independently and as part of a multidisciplinary team.
- Compassionate and patient-centered approach to care.
