The duties below outline the general responsibilities associated with this position. Each of these duties is considered critical to effective department operations and our Mission. It is expected that all staff in this position will have satisfactory attendance and will perform these duties as assigned in a competent, cooperative, and timely manner. The Job Description will be reviewed periodically for accuracy and/or adjusted according to business necessity.
Administers medications to assigned residents according to professional standards, facility policy, and physician's orders. Documents response to medication and medication effectiveness as required.
Ensures the medication cart is cleaned and stocked daily with paper supplies.
Ensures medication orders are picked up and medication is ordered from the pharmacy and delivered in a timely manner.
Performs nursing procedures/treatments to assigned residents according to professional standards, facility policy and physicians orders. Documents response and effectiveness of treatments. Includes vital sign, specimen, and weight collections.
Communicates accurate, up-to-date information to Resident Care Assistants and Memory Care Partners. Provides direction, feedback, and assistance to RCAs and MCPs as needed.
Produces timely accurate and legally compliant documentation of residents' status, changes, and observations in resident chart and communication book.
Educates resident/family members regarding plan of care, medications, procedures and treatments, and participates in Family Meetings as necessary.
Provides continual monitoring of resident's physical, social, and mental status. Promptly reports changes and concerns to LPN Manager.
Maintains and develops work-related knowledge and skills through continuous learning activities, attendance at inservices and workshops, etc.
Completes special projects/tasks in support of effective department functioning as assigned by supervisor.
Minimum Requirements
The following educational minimum requirements must be met by all incumbents:
Must be willing and able to work with older adults in a residential setting with a focus on customer satisfaction.
High School Diploma/GED; Current NYS LPN licensure;
2 years professional nursing experience; previous experience working with senior population/experience in Assisted Living is preferred
Compensation: $45.00 Per Hour
