We are seeking a compassionate, organized, and detail-oriented Case Manager (CDCA) to support individuals receiving substance use disorder (SUD) and mental health services. This role focuses on helping clients navigate care, remove barriers to treatment, and maintain stability in recovery.
Position Summary
The Case Manager will provide care coordination, resource linkage, and recovery support, working closely with clinical staff to ensure clients remain engaged in treatment and progress toward their goals.
We are specifically looking for someone who can serve as a strong support system for clients, demonstrate effective de-escalation techniques, and actively assist individuals throughout their recovery journey.
Key Responsibilities
- Coordinate services for clients in outpatient and IOP programs
- Assist with housing, employment, transportation, and community resources
- Develop and update Individual Service Plans (ISP)
- Monitor client progress and engagement in treatment
- Maintain regular contact with clients (in-person and phone)
- Provide emotional support and crisis de-escalation when needed
- Help clients navigate challenges and remain engaged in recovery
- Collaborate with therapists, prescribers, and peer supporters
- Assist with scheduling appointments and follow-ups
- Support discharge planning and continuity of care
- Complete accurate and timely documentation
Qualifications (REQUIRED)
- Active CDCA (Chemical Dependency Counselor Assistant) certification in Ohio
- Experience working in behavioral health, substance use disorder treatment
- Strong communication and organizational skills
- Experience or training in de-escalation techniques and crisis management
- Ability to work with diverse populations
- Valid driver’s license and reliable transportation
Preferred Candidate
- Experience working with Medicaid populations
- Familiarity with community resources in Columbus, Ohio
- Understanding of recovery-oriented and trauma-informed care
- Ability to manage multiple clients and priorities
- Strong ability to build trust and rapport with clients
Schedule
- Full-time / Part-time available
- Some evenings and weekends may be required
Pay: $16.00 - $22.00 per hour

Experience:
- sustance use disorder/Case Managment: 3 years (Preferred)
