Licensed Practical Nurse (LPN)
The Licensed Practical Nurse (LPN) provides compassionate, high-quality nursing care under the direction and supervision of a Registered Nurse (RN) or licensed provider. The LPN practices within the scope of licensure to support the implementation of individualized care plans, perform focused nursing assessments, administer treatments, monitor patient progress, and promote patient safety while delivering exceptional care in the home setting.
Responsibilities Include:
- Perform focused nursing assessments and follow-up evaluations as assigned.
- Monitor and document patients' physical, behavioral, and emotional conditions, reporting significant changes to the supervising RN or provider.
- Implement nursing interventions and treatments in accordance with the established plan of care.
- Administer medications and treatments as prescribed and within the LPN scope of practice.
- Observe, document, and communicate changes in patient condition, safety concerns, or behavioral issues requiring clinical intervention.
- Collaborate with Registered Nurses, physicians, therapists, CNAs, case managers, and other members of the interdisciplinary care team.
- Reinforce patient and caregiver education regarding medications, wellness, infection prevention, and safety.
- Maintain infection control standards, including proper hand hygiene and use of personal protective equipment (PPE).
- Complete accurate, objective, and timely documentation of assessments, nursing interventions, treatments, and patient responses.
- Follow all agency policies, procedures, state regulations, and clinical direction provided by supervising RN staff.
- Maintain professionalism and provide compassionate, patient-centered care at all times.
- Behavioral health and case management experience is a plus.
- Ability to travel to patients' homes throughout the assigned service area.
- Mileage reimbursement provided.
Qualifications
- Current, unrestricted Licensed Practical Nurse (LPN) license.
- Graduate of an accredited Practical Nursing program.
- Current CPR/BLS certification preferred.
- Strong clinical assessment, communication, and documentation skills.
- Ability to work independently while collaborating effectively with the healthcare team.
- Experience in home health, behavioral health, or case management is preferred but not required.

Pay: $31.90 - $33.46 per hour
