We serve self-referred clients as well as individuals referred through the criminal justice system, probation/parole, DCP&P, healthcare providers, and other community agencies.
Job Overview
We are seeking a motivated and compassionate clinician to join our multidisciplinary treatment team. This position offers the opportunity to provide comprehensive outpatient counseling services while collaborating with healthcare providers, referral sources, and community partners to support clients throughout every stage of the recovery process.
Position Summary
Under the supervision of the Clinical Director, the Outpatient Substance Use Counselor is responsible for providing comprehensive assessment, diagnosis, treatment planning, individual counseling, group counseling, family services, case management, crisis intervention, discharge planning, and clinical documentation for individuals with substance use disorders and co-occurring mental health conditions. The ideal candidate possesses strong clinical judgment, excellent communication skills, a collaborative approach to treatment, and a commitment to providing culturally responsive, trauma-informed, and recovery-oriented care.
Essential Duties and Responsibilities
Clinical Assessment
● Complete comprehensive screening, intake, BPS assessments, and clinical evaluations for all clients.
● Administer the Addiction Severity Index (ASI) and other evidence-based screening/assessment tools.
● Diagnose substance use disorders and co-occurring mental health disorders utilizing DSM-5 criteria.
● Determine the appropriate level of care utilizing the ASAM Criteria.
● Obtain and review previous treatment records and other relevant collateral information.
● Coordinate referrals for psychiatric evaluation, medication-assisted treatment (MAT), medical care, and other community-based services as clinically indicated.
Treatment Planning
● Collaboratively develop individualized, person-centered, recovery-oriented treatment plans.
● Review/update treatment plans according to agency policy, regulatory standards, and client progress.
● Monitor treatment progress and modify interventions based upon ongoing clinical assessment.
● Participate in multidisciplinary treatment team meetings to coordinate client care.
Counseling Services
● Provide evidence-based individual counseling services.
● Facilitate psychoeducational, process, relapse prevention, and recovery-oriented group counseling.
● Conduct family counseling sessions and family education when clinically appropriate.
● Utilize motivational interviewing, cognitive behavioral interventions, and relapse prevention strategies.
Case Management and Care Coordination
● Coordinate care with physicians, psychiatrists, probation officers, courts, hospitals, medication-assisted treatment providers, employers, social service agencies, and other community partners.
● Advocate for clients and assist them in accessing housing, employment, transportation, educational services, recovery supports, and additional community resources.
● Maintain ongoing communication with referral sources while adhering to confidentiality regulations and applicable releases of information.
Continuing Assessment and Discharge Planning
● Reassess client progress throughout treatment using ASAM Criteria and clinical judgment.
● Determine the need for continued treatment, transfer, referral, or discharge.
● Develop comprehensive discharge and continuing care plans to promote sustained recovery.
Documentation
● Maintain accurate, timely, and complete clinical documentation within the agency's EHR.
● Document all assessments, treatment plans, progress notes, collateral contacts, referrals, case management activities, discharge summaries, and other required clinical services in accordance with agency policies and applicable state and federal regulations.
● Ensure all documentation includes required signatures and is completed within established timeframes.
Professional Responsibilities
● Participate in regular clinical supervision and multidisciplinary treatment team meetings.
● Maintain compliance with agency policies, state regulations, licensing standards, and ethical guidelines.
● Participate in staff meetings, training, continuing education, and quality improvement initiatives.
● Maintain professional boundaries and uphold the highest standards of clinical ethics and confidentiality.
● Perform other duties as assigned by the Clinical Director.

* Compensation is commensurate with licensure, certifications, and relevant clinical experience.
Pay: $55,000.00 - $65,000.00 per year
