Duties
- Safely administer medications following prescribed instructions and document all medication activities accurately using Electronic Medical Record (EMR) or Electronic Health Record (EHR) systems such as Epic or Cerner.
- Monitor vital signs including blood pressure, pulse, respiration, and temperature to assess patient health status regularly.
- Assist with activities of daily living such as bathing, dressing, grooming, mobility support, and feeding—especially for individuals with Alzheimer's disease or other forms of dementia.
- Provide specialized care for memory care residents and those receiving hospice or palliative care by implementing tailored routines that promote comfort and dignity.
- Maintain detailed medical documentation in compliance with HIPAA regulations and ensure all records are current and accurate for medical records review.
- Support patients during tube feeding procedures or catheter care with precision and adherence to safety protocols.
- Collaborate with nursing staff and healthcare professionals to update care plans and communicate patient needs effectively.
Skills
- Proven experience in assisted living facilities, nursing homes, hospitals, or home care environments.
- Strong knowledge of medical terminology, physiology, anatomy, and nursing practices related to medication administration and patient care.
- Familiarity with EMR/EHR systems such as Epic or Cerner for accurate documentation of medical records and medication logs.
- Ability to work effectively with individuals who have developmental disabilities or are experiencing cognitive decline due to Alzheimer's or dementia.
- Excellent caregiving skills emphasizing compassion, patience, and clear communication.
- Knowledge of HIPAA regulations to ensure confidentiality and privacy of patient information.
- Experience in hospice or memory care settings is highly desirable to provide specialized emotional support.
