Responsibilities
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary
- Monitor patient health by observing vital signs and reporting changes promptly to healthcare professionals
- Support medication assistance following prescribed instructions while maintaining compliance with HIPAA regulations for patient confidentiality
- Document medical records accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to ensure seamless communication among care teams
- Assist with light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe living environment
Experience
- Prior experience in home health care or assisted living environments preferred; experience in nursing homes or long-term care settings highly valued
- Familiarity with medical terminology, medical documentation practices, and working within HIPAA guidelines is essential
- Hands-on experience with patient observation
- Knowledge of working with individuals with developmental disabilities or disabilities in general enhances your effectiveness in this role
- Demonstrated ability to assist with ADLs and provide compassionate senior care or home & community care services
- Ability to work independently in live-in employment settings or flexible schedules that meet client needs
