Job Summary
We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) / Home Health Aide (HHA) to provide exceptional patient care in home and community settings. Responsibilities
- Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using patient transfer methods and equipment such as Hoyer lifts when necessary
- Monitor and record vital signs such as blood pressure, pulse, respiration, and temperature accurately using EMR (Electronic Medical Record) systems
- Support medication administration following home healthcare regulations and ensuring proper documentation in medical records while maintaining patient confidentiality in compliance with HIPAA standards
- Observe and report changes in patient condition through patient observation techniques; communicate effectively with healthcare teams regarding patient status
- Provide specialized care for clients with dementia, Alzheimer’s disease, or other memory care needs; assist with end-of-life care with compassion and respect
- Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment for clients
- Assist with medical procedures such as catheter care and taking patient personal care assistance as needed while adhering to safety protocols like first aid and basic life support (BLS) or CPR when required
Qualifications
- Valid certification as a Certified Nursing Assistant (CNA) or Home Health Aide (HHA) from an accredited program
- Proven experience working with geriatric patients, individuals with disabilities, or in assisted living environments; experience in nursing homes or hospice care is highly valued
- Knowledge of medical terminology, medication management, and medical documentation practices
- Familiarity with EMR/EHR systems for accurate record keeping and client communication
- Ability to perform heavy lifting safely and use patient transfer equipment such as Hoyer lifts when necessary
- Strong communication skills to effectively engage with clients, families, and healthcare teams
- Knowledge of home health care assistance regulations and best practices for providing compassionate client-centered care
- Certifications in first aid, CPR, and basic life support (BLS) are required or obtainable upon hire
