Position: Palliative Care Manager (LMSW)
Type: Full-Time/Permanent - Direct Hire
Location: Grand Rapids, MI
Shift: Monday–Friday - 8:30 AM – 5:00 PM
Salary: $75,000 – $85,000 annually
Responsibilities:
- Lead the startup and implementation of the palliative care program
- Develop workflows, policies, and operational processes
- Manage program budget and financial performance
- Supervise and support palliative care staff in collaboration with the Medical Director
- Ensure compliance with Medicare, Medicaid, commercial payer, and accreditation requirements
- Assist with provider credentialing and payer contracting
- Oversee referrals, scheduling, documentation, and care transitions
- Drive program growth, quality improvement, and exceptional patient service
- Provide direct social work services during program startup and as needed
- Participate in the hospice Manager On-Call rotation
Qualifications:
- Master's Degree in Social Work (MSW) required
- Licensed Master Social Worker (LMSW) preferred/required per state regulations
- Leadership experience in hospice or palliative care preferred
- Program development experience preferred
- Experience with Electronic Medical Records (EMR) systems
- Proficiency with Microsoft Office
- Excellent leadership, communication, organizational, and collaboration skills
