Qualifications:
- DDS (Doctor of Dental Surgery) or DMD (Doctor of Dental Medicine) degree from an ADA-accredited dental school.
- Board Certified and/or Board Eligible in General Dentistry.
- Current Michigan Dental License and DEA License.
- CPR, BLS, and NPI certifications
