Physical Therapist Assistant (PTA) – Home HealthJob Summary
Navitas Home Health is seeking a compassionate and dependable Physical Therapist Assistant (PTA) to provide skilled therapy services to patients in their homes. Under the direction and supervision of a licensed Physical Therapist, the PTA will carry out established treatment plans designed to improve mobility, strength, balance, safety, and overall functional independence.
The ideal candidate demonstrates strong clinical judgment, excellent communication, accurate documentation, and a commitment to delivering patient-centered care. Candidates must be willing and able to travel to patient residences within a 60-mile radius of the Navitas Home Health office.
Duties and Responsibilities
- Provide physical therapy treatments in accordance with the plan of care established by the supervising Physical Therapist.
- Perform therapeutic exercises, gait and transfer training, balance activities, strengthening, range-of-motion exercises, and other approved interventions.
- Reinforce patient and caregiver education regarding home exercise programs, fall prevention, safe mobility, assistive-device use, and injury prevention.
- Observe and document the patient’s response to treatment, functional progress, tolerance, and any changes in condition.
- Promptly report significant changes, concerns, missed goals, or barriers to progress to the supervising Physical Therapist and appropriate clinical staff.
- Assist with implementing modifications to treatment interventions as directed by the Physical Therapist.
- Safely use, instruct on, and maintain therapy equipment and assistive devices.
- Complete accurate, timely, and compliant visit documentation in the agency’s electronic health record system.
- Participate in care coordination and communicate effectively with physical therapists, nurses, physicians, occupational therapists, caregivers, and other members of the interdisciplinary team.
- Follow the agency’s policies and all applicable state, federal, Medicare, infection-control, and home health documentation requirements.
- Maintain patient confidentiality and comply with HIPAA regulations.
- Participate in staff meetings, case conferences, training, and quality-improvement activities as required.
- Manage an assigned patient schedule and travel efficiently throughout the agency’s service area.
Requirements
- Current and unrestricted Physical Therapist Assistant license in the State of California.
- Graduation from an accredited Physical Therapist Assistant program.
- Current CPR/BLS certification.
- Valid driver’s license, reliable transportation, and current automobile insurance.
- Must be willing and able to travel to patient homes within a 60-mile radius of the Navitas Home Health office.
- Knowledge of anatomy, physiology, therapeutic exercise, gait training, transfer techniques, orthopedics, and medical terminology.
- Ability to recognize and promptly report changes in a patient’s condition.
- Strong clinical documentation, time-management, organizational, and communication skills.
- Ability to work independently in patients’ homes while maintaining communication with the supervising Physical Therapist and interdisciplinary team.
- Familiarity with electronic health record systems and mobile clinical documentation.
- Ability to provide respectful, compassionate care to adults and geriatric patients with varying medical and functional needs.
- Home health experience is preferred.
- At least one year of clinical PTA experience is preferred.
This is a pay-per-visit position requiring travel to patient homes within a 60-mile radius of the Navitas Home Health office.
