Evaluate patients and develop individualized speech therapy treatment plans

Provide in-home speech therapy services

Help patients improve speech, language, communication, cognitive, and swallowing abilities

Educate patients and caregivers on exercises, communication strategies, and safety techniques

Monitor patient progress and adjust care plans as needed

Collaborate with physicians, nurses, and interdisciplinary care teams

Complete accurate and timely documentation
-
Active Texas Speech-Language Pathologist (SLP) license (required)

Home health experience preferred

Strong clinical judgment and patient care skills

Ability to work independently in a home setting

Excellent communication and documentation skills

Reliable transportation & valid driver’s license

Kinnser/WellSky EMR experience (preferred)
