About the Role:
We are seeking a dedicated Licensed Practical Nurse (LPN) to support our multispecialty post‑acute care teams across the Chicago area. In this role, you will play a key part in caring for patients with cardiac, renal, and pulmonary conditions, performing essential diagnostic procedures, and serving as a vital liaison between the center and our organization to ensure seamless communication and high‑quality patient care.
Additionally, this position serves as the lead customer service representative, supporting business development efforts by fostering strong relationships, ensuring a positive experience for patients and partners, and helping advance the growth of our services.
Job Duties & Functions include, but not limited to:
- Perform EKGs and Spirometry testing to support cardiac and pulmonary assessments.
- Collaborate with nurse practitioners, physicians, and facility staff to ensure seamless care.
- Maintain compliance with HIPAA, facility, and company protocols.
What We’re Looking For:
- Current LPN license in the state of Illinois.
- Experience with EKGs and PFT strongly preferred.
- Interest or background in cardiology, nephrology, or pulmonary care a plus.
- Excellent communication and documentation skills.
- A team player who’s passionate about improving patient lives in post-acute and long-term care settings.
Pay: Salary
