We are seeking a compassionate and dedicated RN Case Manager with experience in hospice care to join our team in Houston, TX. The ideal candidate will provide comprehensive case management services to patients receiving hospice care, ensuring quality, continuity, and coordination of care.
- Conduct thorough assessments of patients' physical, emotional, and psychosocial needs in a hospice setting.
- Develop, implement, and update individualized care plans in collaboration with interdisciplinary teams.
- Coordinate and monitor patient care to ensure compliance with hospice regulations and standards.
- Serve as a liaison between patients, families, healthcare providers, and community resources.
- Provide education and support to patients and their families regarding hospice services and end-of-life care.
- Document all patient interactions, care plans, and progress notes accurately and timely.
- Participate in interdisciplinary team meetings and contribute to quality improvement initiatives.
- Current and valid Registered Nurse (RN) license in the state of Texas.
- Current driver's license
- Reliable transportation and valid auto liability insurance.
- Must read, write and comprehend English. Bilingual preferred
- Proven experience as a Hospice Case Manager or in hospice nursing.
- Experience as a registered nurse in a clinical care setting. Hospice experience preferred.
- Strong knowledge of hospice care principles, regulations, and best practices.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work independently and as part of a multidisciplinary team.
- Compassionate approach to patient care and family support.
- Proficiency in Wellsky Consolo EMR is preferred.
Environmental and Working Conditions:
- Works in patients’ homes in various conditions; possible exposure to blood, bodily fluids, and infectious diseases; ability to work a
flexible schedule; ability to travel locally; some exposure to unpleasant weather; PRN emergency calls.
Physical and Mental Effort:
- Prolonged standing and walking required, with ability to lift up to 50 lbs and move patients. Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition; meet patient and family individualized psychosocial needs. Requires eye-hand coordination and manual dexterity.
