Job Responsibilities:
This is a home health position for a Physical Therapist Assistant (PTA). The PT is responsible for creating the Plan of
Care and implementing interventions based on the POC and goals. OT will be responsible for
maintaining contact with the PT and coordinating care between patient and POC. The therapist will
manage their own schedule and drive to patients throughout the day. This job is very autonomous,
perfect for those who don’t enjoy being kept working in a facility all day.
Qualifications:
Degree form an accredited Physical Therapy Program
Current Ohio State License as a Physical Therapist Assistant
Current Ohio Driver’s License and auto insurance
Good organizational and communication skills
We can TRAIN YOU if you do not have home health experience!!

We are well aware of the current standard pay rates in Northwest Ohio. As a result, we offer highly
competitive pay rates for our therapists, rates that cannot be beaten!
Pay: $38.00 - $40.00 per hour
