Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse to provide high-quality care to clients in their homes. The successful candidate will have a solid clinical background, demonstrate exceptional clients care skills, and appreciate the opportunity to work flexible hours while positively impacting clients’ lives.
Responsibilities
- Provide direct patient care in clients' homes under RN supervision and per the established care plan.
- Perform basic nursing care, including wound care, medication administration, and other treatments as ordered.
- Monitor and record vital signs, symptoms, and changes in client condition.
- Assist in implementing and updating individualized care plans as directed by the supervising RN.
- Support and coordinate with CNAs/home health aides as needed.
- Educate clients and family members on basic care, medication adherence, and wellness practices.
- Maintain a clean and safe environment by following infection control protocols.
- Accurately and promptly document all nursing care provided in the client's medical record.
- Report any changes in client condition promptly to the supervising RN.
- Follow all agency policies, procedures, and compliance standards.
Qualifications
- Active LPN license in Georgia.
- At least 1 years of nursing experience (home care preferred but not required).
- Current CPR/BLS certification.
- Excellent organizational and communication skills.
- Strong clinical assessment and critical thinking skills
- Flexible and willing to work in a growing agency environment.
- Reliable transportation for in-home visits
