ROLE AND RESPONSIBILITIES

CLASSIFICATION: Registered Nurse, Adult Foster Care
REPORTS TO: Clinical Manager
REQUIREMENTS/QUALIFICATIONS per AFC Manual:
Qualifications and Competencies: The Registered Nurse – Supervisor must have
- Current Licensure by the Massachusetts Board of Registration in Nursing.
- An Associate Degree in Nursing with 2 years of experience in the direct care of the elderly or disabled adults, OR
- An Associate Degree in Nursing and experience with direct care in Adult Foster Care Program (AFC) with knowledge of AFC Regulations.
- Demonstrated leadership ability.
- Commitment to quality care, and the multidisciplinary team approach.
- Training and experience in working with people diagnosed or undiagnosed mental illness, cognitive and/or intellectual disabilities.
- Strong interpersonal, written, and oral skills.
- Proficiency in MS Office and Outlook 365 and computer program/medical records/database experience. Strong Excel skills a MUST.
- Proficiency with MDS-HC 2.0 and AGD submissions.
- Competence in CPR and First Aid.
- CORI, Physical in the past 12 months, and Annual TB Test.
- Reliable Transportation for trips to multiple locations within Massachusetts to regularly observe staff visits.
PERFORMANCE CRITERIA and Competencies:
- Adherence to AFC regulations and WeCare 365 Policies and Procedures in the supervision of basic and non-skilled nursing service provision by (RN’s), for each AFC member.
- Adherence to AFC regulations and WeCare 365 Policies and Procedures in the supervision of basic Care Management service provision by (CM’s), for each AFC member.
- Annual presentation of proof of required Training for Registered Nurse per AFC Staff regulations to program leadership.
WORKING CONDITIONS:
- Free standing in-home patient environment with exposure to clinical equipment/machinery, medication, cluttered rooms and hallways, noisy environment with frequent interruptions.
- Possible exposure to infectious illness, body fluids and cleaning materials.
- Office and technical equipment such as computers, fax machines and copy machines.
- Frequent commuting in automobile up to 95%.
SPECIFIC ROLE REQUIREMENTS:
Typical duties of a nurse supervisor include supervising a team of nurses, providing leadership and guidance to nursing team, coordinates and maintains the required training and competencies of staff, conducts 1:1 check in, employee 90 day and Annual reviews, performs Quality Check (QC) on required clinical documents and helps nursing staff develop a plan to meet set standards; maintain compliance rates, other daily tasks as assigned by direct manager and senior management.
ROLE SPECIFICS:
Responsible for the overall administration of AFC MassHealth funded program provision, including program planning/development and oversight to all Registered Nurses. This program is managed in accordance with MassHealth AFC guidelines/ COA standards.
- Participates in hiring and termination of nursing staff.
- Provides leadership and supportive guidance to nursing staff.
- Identifies learning opportunities effective coaching and feedback and develop corrective action plans as appropriate.
- Coordinates and maintain the required training and competencies of staff.
- Perform Quality Check (QC) on required clinical documents and help nursing staff develop a plan to meet set standards.
- Create various status reports for Management and Senior Leadership.
- Ensure compliance to AFC regulation and WeCare365 policies and procedures.
- Review and approval of nursing staff Timesheets and Paid Time Off requests.
- Evaluate nursing staff at initial 90 day and continue Annually using standardized evaluation measures. Establish concrete and measurable goals with staff to meet departmental goals that align with WeCare 365’s objectives.
- Conduct 1:1 review meeting with nursing staff and Lead Nurse
- Attend all monthly Massachusetts AFC Council meetings and WeCare 365 meetings.
- Alert Manager of issues and concerns that may create policy changes and achieve conflict resolution.
- Perform other tasks or special projects as assigned by direct Manager and Senior Leadership.

Job Type: Full-time
Pay: $80,000.00 - $85,000.00 per year
