Job Title: RN - Case Manager (Travel)
Location: Hyannis, MA
Type: Contract
Shift & Schedule: Monday–Friday | 40 Hours/Week | 8-Hour Shifts | Rotating Weekend & Holiday On-Call
Position Overview:
Navitas Healthcare, LLC is seeking RN - Case Manager for an exciting Travel opportunity in Hyannis, MA.
Responsibilities
- Lead and oversee daily Case Management department operations
- Supervise clinical resource management and discharge planning processes
- Ensure compliance with CMS regulations and organizational standards
- Develop and implement process improvement initiatives
- Collaborate with interdisciplinary teams to improve patient outcomes and care coordination
- Monitor resource utilization, length of stay, and quality metrics
- Assist in the development and presentation of continuing education programs
- Provide leadership, mentorship, and support to case management staff
- Participate in rotating weekend and holiday on-call coverage
- Maintain accurate documentation and reporting within Epic EMR
Requirements
- Bachelor of Science in Nursing (BSN) required
- Current Massachusetts RN License required
- Minimum 5 years of Acute Care Case Management experience required
- Minimum 2 years of Process Improvement experience required
- Strong knowledge of CMS regulations
- Epic EMR experience required
- Excellent analytical, organizational, interpersonal, and writing skills
- Strong computer proficiency and communication skills
Preferred Qualifications
- Master’s Degree preferred
- Management or supervisory experience preferred
- Certified Case Manager (CCM) certification preferred
- Clinical Documentation Integrity certification preferred
