Cambay Healthcare is hiring a Licensed Bachelor Social Worker (LBSW) for a permanent Case Manager opportunity with a hospital in Salina, Kansas.
Job Responsibilities
- Coordinate patient plans of care and support appropriate levels of care
- Assess patient and family needs and connect them with appropriate services
- Coordinate healthcare and community resources
- Monitor and evaluate patient care plans and outcomes
- Support quality, safe, timely, and efficient patient care
- Work collaboratively with patients, families, healthcare providers, and other departments
Requirements
- Bachelor's Degree in Social Services or related field
- Current Kansas LBSW license – Required
- 2+ years of hospital-based social work experience preferred
- Strong communication, assessment, and care coordination skills
