Are you an RN who enjoys building meaningful relationships with patients and providing one-on-one care? We're seeking an experienced In-Home Services Registered Nurse (RN) to provide skilled nursing care to older adults in their homes while collaborating with an interdisciplinary healthcare team.

What You'll Do
As an In-Home Services RN, you'll coordinate and provide skilled nursing care while serving as a key member of the participant's healthcare team.
Your responsibilities will include:
- Conduct in-home nursing assessments and ongoing participant evaluations
- Develop, implement, and coordinate individualized plans of care
- Perform skilled nursing visits in participants' homes
- Administer medications, treatments, and other nursing interventions
- Educate participants and caregivers on medications, treatments, and disease management
- Monitor participant conditions and communicate changes to providers
- Process physician orders, certifications, recertifications, and discharge summaries
- Coordinate hospital admissions, specialist appointments, and outside services
- Triage participant and staff calls to determine urgency and appropriate follow-up
- Complete timely and accurate clinical documentation
- Assess participants for changes in condition and recommend appropriate levels of care
- Coordinate services with home health, therapy, and community resources
- Participate in interdisciplinary care conferences and quality improvement initiatives
- Supervise LPNs and Home Health Aides/CNAs in accordance with regulatory requirements
- Ensure high-quality care through ongoing participant and caregiver follow-up
What We're Looking For:
Required Qualifications
- Active Colorado Registered Nurse (RN) license
- Current BLS and First Aid certification through the American Heart Association or American Red Cross (required before hire)
- Minimum 1 year of experience caring for frail or elderly adults
- Valid driver's license, reliable personal vehicle, and current auto insurance
- Ability to travel throughout the service area for participant home visits
- Strong assessment, communication, and care coordination skills
