About the Role Fairview Skilled Rehab and Nursing Home is looking for a compassionate and experienced Physician to provide medical care to our short-term rehab and long-term nursing residents.
Responsibilities
Conduct admissions, routine visits, and urgent evaluations
Manage individualized care plans with the interdisciplinary team
Order and interpret labs, imaging, and diagnostics
Prescribe and monitor medications and treatments
Ensure documentation meets Medicare/Medicaid standards
Communicate with residents, families, and outside providers
Requirements
MD or DO with a valid New York State license
Board certified/eligible in Internal Medicine, Family Medicine, or Geriatrics
Long-term care or geriatric experience preferred
Strong communication skills and EHR proficiency
