A respected nonprofit human services organization in the Savannah and surrounding area is seeking a dedicated, compassionate, and highly organized Case Manager to join our team. This position plays a critical role in supporting vulnerable populations through individualized service planning, advocacy, resource coordination, crisis intervention, and housing stabilization services.

Position Summary
The Case Manager is responsible for providing comprehensive case management services to program participants experiencing housing instability, crisis situations, or other barriers to self-sufficiency. The ideal candidate is empathetic, resourceful, organized, and able to balance documentation, compliance, and direct client engagement effectively.
This role requires strong communication skills, professionalism, sound judgment, and the ability to work independently while maintaining a high standard of client-centered care.
Key Responsibilities
- Conduct client assessments and develop individualized service plans
- Maintain ongoing contact and engagement with participants
- Coordinate referrals to community resources and supportive services
- Assist clients with housing stabilization and goal achievement
- Maintain accurate and timely case documentation
- Monitor participant progress and compliance with program requirements
- Participate in interdisciplinary staffing and team collaboration
- Advocate on behalf of clients with community partners and providers
- Assist with crisis intervention and conflict resolution as needed
- Ensure compliance with agency policies, funding requirements, and confidentiality standards
Qualifications
- Bachelor’s degree in Social Work, Human Services, Psychology, Sociology, or related field preferred
- Relevant case management or human services experience may substitute for education
- Strong organizational and time management skills
- Ability to manage multiple priorities in a fast-paced environment
- Excellent written and verbal communication skills
- Experience working with vulnerable populations preferred
- Knowledge of community resources and supportive service systems preferred
- Proficiency with electronic documentation and case management systems
- Valid driver’s license and reliable transportation preferred

Pay: $20.00 - $23.00 per hour
