Job Overview
We are seeking a dedicated full time and part time On-Call Nurse - RN to join our compassionate team. This role offers flexible, on-demand nursing support to ensure seamless patient care during after-hours or when regular staff are unavailable. The ideal candidate will possess a broad skill set , with experience working in diverse healthcare settings, including hospice, hospital, and long-term care environments. This position emphasizes patient-centered care, clinical excellence, and effective communication.
Responsibilities
- Provide comprehensive nursing assessments and interventions for patients in various settings, including hospice, home care, assisted living, and inpatient facilities.
- Respond promptly to urgent patient needs during on-call hours, including managing acute pain.
- Collaborate with interdisciplinary teams to develop and update individualized care plans aligned with patient goals and clinical standards.
- Maintaining HIPAA compliance.
- Assist with triage by evaluating symptoms, prioritizing care needs, and coordinating emergency responses when necessary.
- Educate patients and families about disease management, medication protocols, and advanced directives to promote informed decision-making.
Qualifications
- Valid RN license with current registration; additional certifications in Basic Life Support (BLS) required.
- Familiarity with MatrixCare for accurate documentation.
- Strong assessment skills with the ability to evaluate vital signs, and perform physical examinations.

- Bachelor’s or Associates degree, with one (1) year of hospice care experience preferred.
- Must exhibit the ability to interact well with a great diversity of individuals and the ability to organize and execute Wren Hospice policies and procedures.
- Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.

Work Location: On the road
