Position Summary
At Foundation X, the Psychosocial Rehabilitation (PSR) Specialist provides structured, skill-building services to clients in community-based settings. Services focus on restoring, improving, and maintaining daily living, social, and behavioral health functioning.
The PSR Specialist works directly with clients in their homes and communities, implementing individualized treatment plans and supporting clients in achieving stability, independence, and improved quality of life.
Minimum Qualifications
- Must have a minimum of two (2) years of experience in behavioral health, specifically providing community-based, field services
- Bachelor’s degree in a behavioral health-related field preferred (Psychology, Sociology, Social Work, Counseling, Education or related field )
- High school diploma required (must meet Louisiana Medicaid and LDH requirements for PSR services)
- Valid driver’s license and reliable transportation
- Ability to pass background check and maintain compliance with all state and regulatory requirements
Core Responsibilities
Direct Service Delivery
- Provide PSR services in community settings (home, school, community locations)
- Implement individualized treatment plans as developed by licensed clinicians
- Assist clients in developing skills related to:
- Daily living (hygiene, routine, organization)
- Social interaction and communication
- Emotional regulation and coping strategies
- Behavioral management
- Document all services accurately and within the required 48-hour timeframe
Client Support & Monitoring
- Monitor client progress toward treatment goals
- Identify changes in behavior, functioning, or risk factors
- Communicate concerns to supervisors and clinical staff immediately
- Participate in crisis intervention when necessary
Documentation & Compliance
- Complete progress notes, service logs, and required documentation in compliance with:
- Louisiana Department of Health (LDH)
- Medicaid guidelines
- The Joint Commission standards
- Ensure all documentation supports medical necessity and service delivery
Collaboration & Communication
- Work closely with CPST staff, therapists, MST team, and leadership
- Attend required staff meetings, supervisions, and trainings
- Communicate effectively with families, caregivers, and referral sources
Productivity & Performance Expectations
- Maintain consistent weekly service delivery as assigned
- Meet productivity standards set by Foundation X
- Ensure all documentation is submitted within required timelines
- Maintain professional conduct in all client and community interactions
Work Environment
The PSR Specialist primarily works in community-based settings and is expected to travel regularly to client locations. This role requires strong time management, independence, and the ability to manage a flexible schedule.
The position involves direct interaction with clients who may present with behavioral, emotional, or psychiatric challenges. The PSR Specialist must be able to remain calm, professional, and solution-focused in high-stress situations.

Supervision Structure
- Reports to: Program Director / Clinical Supervisor

Pay: $18.00 - $25.00 per hour
