Champion Care is seeking a driven and relationship-focused Hospital Liaison to support our skilled nursing and post-acute care centers throughout the Knoxville market.
The Hospital Liaison is responsible for building strong relationships with hospitals and healthcare referral partners, coordinating skilled nursing admissions, and driving census growth. You will work closely with hospital case managers, discharge planners, social workers, physicians, and facility admissions teams to help patients transition smoothly from the hospital to the appropriate skilled nursing or rehabilitation setting.
This is an excellent opportunity for someone with experience in skilled nursing, hospital relations, healthcare admissions, healthcare marketing, business development, case management, or post-acute care who enjoys building relationships and driving results.
Build and maintain strong relationships with hospital case managers, discharge planners, social workers, physicians, and other referral partners
Develop and grow hospital referral relationships throughout the Knoxville market
Review patient referrals and coordinate admissions into skilled nursing and rehabilitation centers
Drive admissions and census growth through consistent hospital outreach, relationship development, and referral follow-up
Coordinate smooth transitions of care from hospitals to skilled nursing and post-acute care settings
Partner with facility administrators, admissions teams, and clinical leadership to support occupancy goals
Maintain a strong presence at hospitals and with key referral partners throughout the market
Represent Champion Care at hospital meetings, networking events, and community outreach opportunities
Identify new referral sources and opportunities for market growth
Track referral activity, admissions opportunities, and market trends
Experience in skilled nursing, post-acute care, hospital liaison, healthcare admissions, healthcare marketing, business development, or case management preferred
Established hospital or healthcare relationships within the Knoxville market are highly preferred
Strong communication, networking, sales, and relationship-building skills
Ability to work independently and manage multiple priorities
Knowledge of skilled nursing, rehabilitation, or post-acute care preferred
Clinical healthcare background is a plus, but not required
Ability to travel throughout the Knoxville market
