General Description:
The Case Manager for Family Empowerment Services (FES) reports to the FES Supervisor for clinical guidance and consistent supervision. The FES Case Manager will serve as a liaison between DHS and the families. Each FES Case Manager will have an individual caseload of 15 cases. The Case Manager will be responsible for performing the duties below for each family.
Duties and Responsibilities:
- Identifies existing support services, assesses family’s needs, and coordinate new services to ensure a structured support system is in place for the family;
- Makes bi-monthly visits in the home and coordinate and ensures that enhanced home services are provided;
- During home visits provide in-home counseling, parenting support, life skills, services planning, and referrals;
- Work with the family to develop a Service Plans and monitor and document progress on each case;
- Prepares and submit timely Contact Events Notes to DHS;
- Ensure that Service Plan goals are achievable and met within the 90 days of the FES service period;
- Writes and maintains up-to-date case notes;
- Provides translation and interpretation, if necessary;
- Reports weekly activities to immediate supervisor;
- Attends weekly supervision, staff training, and meetings;
- Complete daily time sheets and submits weekly work schedules and time sheets
Qualifications:
- Bachelor degree from an accredited college or university with a degree in social work, education, sociology, humanities or related field.
- Mandatory - official transcript from identified accredited university reflecting Bachelor's degree awarded (prior to official hire date)
- Two years of experience in human services, preferably child welfare.
- Knowledge of local social service system preferred.
- Proficiency in Microsoft Office.
- Ability to work within and relate to a multicultural environment.
- Ability to travel throughout Philadelphia (Reliable Vehicle Required)
- Criminal background check, FBI and Child Abuse Clearance
