Job Overview
We are seeking a dedicated and compassionate Physical Therapist (PT) to join our team. This role requires a strong understanding of physiology and effective discharge planning to ensure optimal patient outcomes. As a PT, you will play a crucial role in enhancing the quality of life for our patients through tailored treatment plans and ongoing support.
Responsibilities
- Conduct comprehensive assessments of patients’ physical abilities and needs.
- Develop individualized treatment plans based on patient evaluations and goals.
- Implement therapeutic exercises and modalities to improve mobility, strength, and function.
- Provide education to patients and their families regarding treatment options and home care strategies.
- Collaborate with interdisciplinary teams to coordinate patient care effectively.
- Monitor patient progress and adjust treatment plans as necessary.
- Assist with discharge planning to ensure smooth transitions from clinic or home health settings.
- Maintain accurate documentation of patient care activities and progress.
Requirements
- Valid state licensure as a Physical Therapist.
- Strong knowledge of physiology, anatomy, and physical therapy principles.
- Experience in home health, outpatient settings, or geriatrics is preferred.
- Excellent communication skills for effective interaction with patients, families, and healthcare teams.
- Ability to develop rapport with patients while providing compassionate care.
- Familiarity with electronic health record (EHR) and OASIS is must.
- Commitment to ongoing professional development and staying current with industry best practices.
