We are a family owned Waiver Provider for Summit, Stark, Columbiana, Franklin and Cuyahoga Counties. Our main focus is creating a supportive environment for our staff members and providing active and fulfilling lives for the Individuals we serve. The current position for a Healthcare Coordinator is Part time, but Full Time could be discussed.
Responsibilities include:
- Managing appointment scheduling for adults with Developmental Disabilites
- Accompanying Individuals to appointments and clearly addressing current concerns
- Relaying important health information to supervisors and teams including timely in-servicing
- Communicate effectively with clients, families, and team members.
- Attend training sessions and staff meetings.
- Building a strong rapport with Individuals to support them well at appointments
- Ensuring that medication changes are updated properly and relayed to necessary team members

Who We’re Looking For:
- Degree preferred. A strong medical background such as MA preferred. Candidate must be friendly and confident talking with Dr's and nursing staff.
- Highly compassionate, caring, and dedicated individuals who truly enjoy making connections with others. If you an extroverted people person, this job is for you!
- Those who are eager to help and enhance the lives of people with developmental disabilities
- Candidates who value communication and are excited to work with a supportive team.
- Excellent references are a must, as we believe in building trust and strong relationships with both our team and the individuals we serve.
- Must have excellent driving record and be comfortable operating large vans (training provided)
- Current Medication Administration Certificate preferred
