As a Registered Nurse Case Manager (RNCM) you will make a difference in the lives of our patients. You will utilize your leadership skills in coordinating care and provide home-based nursing care for patients as directed by an attending physician.

REQUIREMENTS:
- Registered Nurse with current license in the state of employment
- Minimum one-year experience as an RN in an acute care setting. Home care experience preferred
- Valid driver's license and auto insurance in your name as a driver
Pay: $68,000.00 - $81,377.00 per year
