Job Summary
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong understanding of patient care, excellent communication skills, and the ability to work independently while adhering to established protocols and guidelines.
Duties
- Conduct comprehensive assessments of patients' health status, including physical, emotional, and social needs.
- Develop and implement individualized care plans based on patient assessments and collaboration with healthcare teams.
- Administer medications and treatments as prescribed, including catheter care and sterile processing.
- Monitor patients for changes in condition, documenting observations accurately in electronic health records.
- Provide education and support to patients and their families regarding health management, including dementia care strategies.
- Assist with emergency medicine procedures when necessary, ensuring patient safety at all times.
- Ensure compliance with all regulatory requirements and best practices in home health nursing.
Qualifications
- Current Registered Nurse (RN) license in good standing.
- Experience in home health or community nursing is preferred.
- Skills in infant care are desirable for working with pediatric patients.
- Strong organizational skills with the ability to manage multiple tasks effectively.
- Excellent interpersonal skills to foster positive relationships with patients and their families.
- Ability to work independently while being part of a collaborative healthcare team.
