Meadowbrook Post Acute is seeking a compassionate, organized, and dedicated Social Services Designee to join our interdisciplinary team. We are committed to providing exceptional short-term rehabilitation and long-term skilled nursing care while enhancing the quality of life for every resident.
As our Social Services Designee, you will play a vital role in supporting residents and their families through the admission process, care planning, discharge planning, and ongoing psychosocial support.
Responsibilities
- Complete psychosocial assessments for residents upon admission and as needed.
- Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team.
- Assist residents and families with adjustment to the facility and provide emotional support.
- Coordinate discharge planning to ensure safe and successful transitions.
- Facilitate referrals to community resources and outside agencies.
- Advocate for residents' rights, dignity, and quality of life.
- Assist residents and families with advance directives and other social service needs.
- Participate in care conferences and Quality Assurance/Performance Improvement (QAPI) activities.
- Maintain accurate, timely, and compliant documentation in accordance with state and federal regulations.
- Collaborate with nursing, therapy, dietary, admissions, and other departments to meet resident needs.
- Support residents with concerns related to financial resources, transportation, housing, insurance, and community services when appropriate.
Qualifications
- Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or a related field preferred.
- Social Services experience in a skilled nursing facility or healthcare setting preferred.
- Knowledge of Medicare, Medi-Cal/Medicaid, discharge planning, and long-term care regulations is preferred.
- Strong communication, organization, and interpersonal skills.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
- Compassionate, professional, and resident-centered approach to care.
- Proficiency with electronic medical records and Microsoft Office applications.
