Job Overview
The Licensed Practical Nurse (LPN) is responsible for providing quality nursing care to residents in accordance with physician orders, facility policies, state regulations, and federal guidelines. The LPN works under the supervision of the Director of Nursing and/or Registered Nurse and collaborates with the interdisciplinary team to ensure residents receive safe, compassionate, and person-centered care.
This job is for 3pm-11pm Monday-Friday
Resident Care
- Administer medications and treatments as ordered by the physician.
- Monitor residents for changes in condition and promptly report findings to the physician, RN, and/or Director of Nursing.
- Complete nursing assessments as assigned and document findings accurately.
- Assist with admissions, transfers, discharges, and readmissions.
- Provide wound care and other skilled nursing services within the scope of practice.
- Respond appropriately to medical emergencies and changes in resident condition.
Documentation
- Accurately complete all required documentation in the electronic medical record (EMR).
- Document medications, treatments, physician notifications, incidents, and resident responses timely and thoroughly.
- Maintain compliance with facility, state, and federal documentation requirements.
Communication
- Communicate effectively with residents, families, physicians, pharmacies, and healthcare providers.
- Participate in shift reports, care plan meetings, and interdisciplinary team meetings.
- Provide guidance and direction to Certified Nursing Assistants (CNAs) and Certified Medication Aides (CMAs).
Regulatory Compliance
- Follow all Oklahoma State Department of Health regulations and CMS guidelines.
- Adhere to infection control practices and facility safety protocols.
- Maintain resident confidentiality in accordance with HIPAA regulations.
- Participate in quality assurance and performance improvement activities as assigned.
Leadership
- Supervise and support CNAs and CMAs during assigned shifts.
- Ensure resident care tasks are completed appropriately and timely.
- Assist with staff training and mentoring as needed.
- Promote teamwork and a positive work environment.
QualificationsRequired
- Current Oklahoma Licensed Practical Nurse (LPN) license in good standing.
- Current CPR certification.
- Ability to pass required background checks.
- Strong communication and organizational skills.
- Ability to work independently and as part of a team.
Preferred
- Long-Term Care or Skilled Nursing experience.
- Experience with electronic medical records (EMR). Point Click Care
- Experience supervising nursing assistants and medication aides.
Physical Requirements
- Ability to lift, transfer, and reposition residents as needed.
- Ability to stand, walk, bend, and reach throughout the shift.
- Ability to respond quickly in emergency situations.
