Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner.
- Administer medications and injections accurately following physician orders while adhering to HIPAA regulations and sterile technique protocols.
- Assist with medical procedures such as IV infusion, catheterization, tube feeding, wound suturing, and spinal tap procedures.
- Monitor patients' conditions closely, including managing acute pain, respiratory support with ventilators, and airway management in emergency or ICU settings.
- Conduct patient triage in urgent care or emergency room environments to determine priority and appropriate level of care.
- Collaborate with multidisciplinary teams for discharge planning, case management, and coordination of home & community care services.
- Maintain meticulous medical documentation of patient care activities, medication administration records, and diagnostic evaluations ensuring compliance with ICD coding standards like ICD-9 and ICD-10.
Requirements
- Valid state licensure as a Licensed Practical Nurse (LPN) with current certification.
- Proven experience in diverse healthcare environments such as hospitals (including Level I Trauma Centers), nursing homes, hospice care, or outpatient clinics.
- Demonstrated proficiency with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks.
- Strong knowledge of physiology, anatomy, medical terminology, and diagnostic evaluation techniques.
- Experience working with individuals with disabilities or special needs including dementia care (Alzheimer’s and memory care), behavioral health challenges, or developmental disabilities.
- Ability to perform clinical skills such as phlebotomy, venipuncture, sterile processing techniques, and basic life support (BLS).
- Excellent communication skills for patient assessment discussions and collaboration with healthcare teams.
- Flexibility to work in fast-paced environments like ICU, emergency medicine, or urgent care settings; travel nursing experience is a plus.
