Position Summary:
An Outpatient RN Case Manager is responsible for coordinating and managing patient care across the outpatient setting to ensure high-quality, cost-effective, and patient-centered care. The RN Case Manager collaborates with physicians, specialists, social workers, insurance providers, and other healthcare professionals to develop individualized care plans, coordinate services, facilitate transitions of care, and improve patient outcomes.
Roles and Responsibilities:
- Conduct comprehensive patient assessments, including medical, psychosocial, and discharge planning needs.
- Develop, implement, and update individualized care plans based on patient goals and clinical needs.
- Coordinate outpatient services, specialist referrals, diagnostic testing, therapies, and follow-up appointments.
- Collaborate with physicians, nurses, social workers, therapists, pharmacists, and community resources to ensure continuity of care.
- Educate patients and families regarding diagnoses, medications, treatment plans, disease management, and preventive care.
- Monitor patient progress and adjust care plans as needed.

Requirements:
- Active CA RN license
- BLS(AHA)/CPR
- Minimum 2 years of Case Management experience preferred
- 1 year of prior RN experience REQUIRED (Clinic, Primary Care, Pre/Post, Home Health settings. (Prefferd)
