A family-owned healthcare organization is seeking a dedicated and organized ISP Nurse (RN) to support our continuum of care across three Skilled Nursing Facilities and one Intermediate Care Facility (ICF).
This position plays an important role in coordinating individualized resident care, maintaining regulatory compliance, and collaborating with interdisciplinary teams to ensure residents receive quality, person-centered services.

Coordinating and maintaining Individual Service Plans (ISPs) in accordance with applicable regulations
Collaborating with interdisciplinary team members to develop, review, and update resident care plans
Participating in resident assessments and care conferences
Monitoring documentation to ensure accuracy, completeness, and regulatory compliance
Working closely with nursing leadership, social services, therapy, and other departments to support resident outcomes
Assisting with regulatory surveys, audits, and quality improvement initiatives
Educating staff on care planning requirements and best practices
Supporting consistent communication between residents, families, and the interdisciplinary team
Current Registered Nurse (RN) license
Previous experience in long-term care, skilled nursing, or intermediate care preferred
Knowledge of care planning and regulatory requirements
Strong organizational and documentation skills
Excellent communication and collaboration abilities
Ability to manage multiple priorities across multiple facilities
Commitment to providing compassionate, resident-centered care
