JOB SUMMARY: Assumes responsibility for individual medical case management of agency individuals on a case-by-case basis. Responsible for ensuring the quality of medical care provided meets DDD and Agency standards while working cooperatively with the Medical Case Manager, Director of Residential and the Executive Director to uphold agency and licensing regulations.
SALARY RANGE: $35.50/hour
REQUIREMENTS: Must have valid license as a Licensed Practical Nurse in New Jersey; 2 years’ experience in the nursing field; ability to determine the needs and abilities of the individuals and solve problems systematically; must be at least 18 years old, have a valid NJ Driver’s License with an acceptable record of safe driving.
