Good Samaritan Hospice, an entity of Concordia Lutheran Ministries, is seeking a Registered Nurse Case Manager to provide comprehensive end-of-life care and support to our patients and their families in their homes in the Moon Twp/McKees Rocks/South Hills areas.

As a Hospice Registered Nurse Case Manager RN, you will be an integral part of the interdisciplinary team that collaborates with our physicians, social workers, spiritual and volunteer coordinators to ensure the comfort and dignity of our patients in their final stages of life.
We have both full and part time Registered Nurse Case Manager positions available. Schedule includes working one weekend a month and an on-call rotation.

QUALIFICATIONS FOR THE Hospice Registered Nurse Case Manager RN :
- Current RN license in the state of Pennsylvania
- Demonstrates excellent clinical and documentation skills
- Excellent organizational skills with attention to detail
- Outstanding communication skills (both written and verbal)
- Reliable independent means of transportation with a valid Pennsylvania driver’s license
RESPONSIBILITIES FOR THE Registered Nurse Case Manager RN position:
- Assess and monitor the physical, emotional, and spiritual needs of our hospice patients and create individualized care plans.
- Educate patients and their families about the disease process, care options, and end-of-life decisions.
- Provide pain management and symptom control measures to alleviate discomfort and improve quality of life.
- Advocate for our patients, ensuring their wishes and preferences are respected and honored.

Starting salary is around $90,000 and up - Compensation will be determined based on years of experience, applicable certifications and by the number of visits completed each week.
