Job Summary
Patient Navigation & Linkage to Care
- Conduct patient intake and eligibility screening for PrEP, PEP, STI, and related prevention services.
- Coordinate linkage to medical providers for PrEP initiation and follow-up care.
- Schedule medical appointments, laboratory services, and follow-up visits.
- Track patient progress from referral through successful enrollment and retention in care.
- Maintain ongoing communication with patients to support treatment adherence and appointment compliance.
- Provide reminder calls, texts, and outreach to patients who miss appointments.
- Assist patients with accessing primary care, behavioral health, substance use treatment, housing, transportation, and other supportive services as needed.
- Coordinate referrals to community-based organizations and social service agencies.
PrEP Program Coordination
- Educate patients regarding PrEP, PEP, HIV prevention strategies, STI prevention, and medication adherence.
- Explain available medication options, laboratory requirements, and follow-up schedules.
- Assist patients with enrollment into manufacturer assistance programs, copay assistance programs, and insurance coverage programs.
- Monitor and track medication start dates, refill dates, and follow-up requirements.
- Coordinate with pharmacies to ensure timely medication delivery and refill processing.
- Support provider teams in maintaining continuity of care for PrEP patients.
Patient Services
- Assist patients with completion of required forms, consents, and program documentation.
- Verify insurance coverage and assist with insurance navigation when appropriate.
- Address barriers impacting patient engagement in care.
- Provide culturally competent, trauma-informed, and non-judgmental services to diverse populations.
- Serve as a patient advocate while maintaining professional boundaries and confidentiality.
Outreach & Community Engagement
- Participate in outreach events, testing events, health fairs, and community education activities.
- Develop and maintain relationships with referral partners, community organizations, pharmacies, and healthcare providers.
- Promote awareness of HIV prevention services and available resources.
- Support initiatives focused on populations disproportionately impacted by HIV.
Documentation & Compliance
- Maintain accurate and timely documentation within the electronic health record (EHR) and other required databases.
- Complete all required reports, tracking logs, and grant-related documentation.
- Maintain strict confidentiality and HIPAA compliance.
- Ensure compliance with organizational policies, Department of Health requirements, and funding agency standards.
- Participate in quality improvement activities and program audits.
Administrative Responsibilities
- Maintain patient tracking systems and referral logs.
- Assist with data collection and program reporting.
- Participate in staff meetings, trainings, and professional development activities.
- Perform other duties as assigned.
Job Type: Full-time
Pay: $16.00 - $18.00 per hour
