First State Home Health & Hospice Care is seeking a compassionate and dependable Certified Nursing Assistant (CNA) to provide exceptional care to patients in their homes and hospice settings. This position is ideal for someone who enjoys working independently while making a meaningful difference in the lives of patients and families.
Responsibilities
- Provide personal care including bathing, dressing, grooming, and hygiene assistance.
- Assist patients with ambulation, transfers, and mobility.
- Observe and report changes in patient condition to the supervising nurse.
- Provide emotional support and companionship to patients and families.
- Assist with light housekeeping related to patient care.
- Follow the patient's individualized plan of care.
- Complete accurate documentation after each visit.
- Maintain patient privacy and confidentiality.
- Comply with all agency policies and infection control standards.
Qualifications
- Current Michigan Certified Nursing Assistant (CNA) certification.
- Current CPR certification.
- Valid driver's license and reliable transportation.
- Current automobile insurance.
- Ability to travel throughout the assigned service area.
- Excellent communication and interpersonal skills.
- Previous home care or hospice experience preferred but not required.
