JOB SUMMARY
At PARC Hospice, we pride ourselves on providing compassionate, patient-centered care that consistently earns high praise from our clients and their families. As a Registered Nurse (RN) at PARC Hospice, you will:
- Provide Patient-Centered Care: Perform patient visits, assessments, and interventions across various settings, including private homes, assisted living communities, and skilled nursing facilities.
- Collaborate with Interdisciplinary Teams: Communicate and collaborate effectively with social workers, chaplains, volunteers, and CNAs to support comprehensive, holistic care.
- Manage Patient Care Independently: Take ownership of patient outcomes, balancing clinical visits, documentation, and team meetings while receiving support from a dedicated care team.
- Support Continuity of Care: Participate in the on-call rotation to ensure patients receive continuous, high-quality care.

QUALIFICATIONS
- Active Registered Nurse (RN) license in Michigan (required)
- Minimum of 3 years of nursing experience (required)
- Home Health experience (preferred)
- Hospice or palliative care experience (preferred, but not required)
- Strong organizational, communication, and interpersonal skills
- Reliable transportation for travel within the service area
