Full Job Description
We are seeking a Registered Nurse- Home Health per diem. To join our amazing team! You will be responsible for delivering high-quality care to assigned patients in a home health environment.
Job responsibilities include but are not limited to:
- Assess patient status to provide treatment and care in the comfort of their home
- Routinely administer prescribed medication, treatments, and therapies
- Work and communicate effectively in a multi-disciplinary team
- Patient medical supply management
- Patient health promotion and teaching
- Training of patient family members as needed
Qualifications:
Candidates of a Registered Nurse
- Has current and unrestricted California state license to practice as a Registered Nurse/RN
- Associates degree in nursing (ADN) or Bachelor’s degree in nursing (BSN)
- Basic Life Support (BLS)-CPR required
- Ability to apply sound judgment to work independently and with minimal supervision
- Knowledge of relevant policies, procedures, and equipment necessary to practice Home Health Care
- Ability to Chart Electronically
- Physical and mental ability to support changes in work assignments related to patient needs
- Must have a minimum of one (1) year of experience in Hospice, a valid driver’s license, and reliable transportation to drive to patients home
