Job Title: Home Health RN
Assignment Length: 13 weeks with possible extension
Location: Newton, MA
Shift: 8am-4:30pm; Must be willing to work weekends. Weekend rotations are once every 6 weeks.
Pay Rate: $70/hour
Job Summary:
As a Home Health RN, responsibilities not limited to assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. Patient care is delivered within organizational philosophy, policy and standards of community health practice. Also required to perform admission processes and case management responsibilities in accordance with the Home Care’s policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications.
Experience:
- 1 year Certified Home Care experience - a Must!!
- 1 year Case Management experience- a Must!!
Job Requirements:
- RN License in Massachusetts
- Reliable, independent means of transportation to make home visits.
- Fully Vaccinated against Covid-19
