Job Overview
We are seeking a dedicated Licensed Practical Nurse (LPN) to provide high-quality patient care across various healthcare settings. The ideal candidate will possess strong clinical skills, experience with diverse patient populations, and proficiency in electronic health record (EHR) systems. This role involves delivering compassionate nursing services, performing assessments, and supporting treatment plans to promote optimal health outcomes. The LPN will work closely with interdisciplinary teams to ensure compliance with healthcare standards and regulations, including HIPAA and NCQA standards. Experience in specialized areas such as critical care, geriatrics, or emergency medicine is highly valued.
Duties
- Conduct comprehensive patient assessments including vital signs, physical examinations, and medical history documentation using EMR/EHR system.
- Administer medications and injections accurately following physician orders and safety protocols.
- catheterizations, phlebotomy, and sterile procedures adhering to aseptic techniques.
- Assist with diagnostic evaluations including laboratory specimen collection, processing, and medical imaging preparations.
- tube feeding, and wound suturing as required for specialized care units such as ICU or surgical wards.
- Provide patient education on medication management, pain control (including acute pain management), nutrition, and chronic disease management like dementia or Alzheimer's care.
- Support discharge planning and case management activities to ensure seamless transition of care for post-acute or home care patients.
- Ensure compliance with infection control protocols and maintain accurate medical documentation in accordance with HIPAA regulations.
- Collaborate with physicians and other healthcare professionals in emergency room settings, Level I Trauma Centers, or urgent care environments for triage and acute care delivery.
- Work with diverse populations including , adult, elderly (geriatrics), or individuals with developmental disabilities health needs.
Qualifications
- Current valid license as a Licensed Practical Nurse (LPN).
- Proven experience in medical-surgical units, ICU/CCU/ICU settings (including Level I Trauma Centers), emergency rooms, or specialty areas such as cardiology, geriatrics.
- Proficiency with EMR/EHR systems such as Epic, Cerner, eClinicalWorks, or Athenahealth.
- Strong understanding of physiology, anatomy, pharmacology, and medical terminology relevant to patient assessment and treatment planning.
- Ability to perform invasive procedures like venipuncture, IV insertion/infusion therapy, catheterization (including Foley catheters), spinal taps, gastric lavage, or dermal fillers if applicable.
- Excellent communication skills for effective patient education and interdisciplinary collaboration.
- Ability to work independently while maintaining high standards of patient safety and confidentiality in compliance with HIPAA regulations.
