Responsibilities:
- Provide emotional, social, and psychological support to residents, addressing their physical, emotional, and spiritual needs
- Develop and implement individualized care plans in collaboration with residents, families, and healthcare professionals
- Coordinate and facilitate resident outings, social events, and activities, ensuring a sense of community and social engagement
- Maintain accurate and detailed records of resident care, progress, and interactions
- Communicate effectively with residents, families, and staff members to ensure a seamless care experience
- Collaborate with interdisciplinary teams to address resident needs and concerns
- Participate in resident assessments, care planning, and care delivery
Requirements:
- High school diploma or quivalent required;
degree in social work, psychology, or related field preferred
- 1-2 years of experience in social services or related field; passion for working with seniors a must
- Strong communication, interpersonal, and problem-solving skills
- Ability to maintain confidentiality and discretion when handling sensitive information
- Must be able to lift up to 25 pounds and stand for extended periods
