We are expanding our footprint and seeking compassionate Physical Therapists to provide home-based physical therapy services. Our therapists contribute to improving mobility and enhancing the quality of life to our members.
Physical Therapist Responsibilities:
- Travel to and from patients' homes, providing physical therapy as ordered by a physician in accordance with the designated plan of care.
- Design treatment plans that achieve results through personalized, 1-on-1 individualized care.
- Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
- Consult with patients to learn about their physical condition and symptoms and review their medical history and referrals from doctors or surgeons
- Diagnose movement dysfunction by listening to patients and observing them as they move about
- Develop quality, personalized, evidence-based care intervention programs that improve outcomes
- Use hands-on therapy such as massage to ease patients’ pain and provide stimulation to promote healing
- Maintain patient records by keeping track of goals and progress while adapting treatment plans accordingly
- Instructs family members, Home Health Aides, and other caregivers in appropriate physical therapy techniques and what to expect from the recovery process
Qualifications:
- Current Physical Therapy Licensure in NY State
- Graduate of an accredited PT program
- Proven work experience as a physical therapist
- Intensive education and clinical expertise
- Ability to manage patients with different types of personalities
- Current knowledge of treatment practices
- Excellent interpersonal communication skills
Pay: $80.00 - $95.00 per hour
