The Arc Greater Hudson Valley, a not-for-profit agency with a mission of supporting people with unique abilities to live as valued and contributing members of the community, is currently looking for dedicated and team-oriented individuals to help fulfill this mission.
Job Description:
The Community Habilitation Provider is responsible for providing and training in goal-directed, meaningful activities. In addition, the Provider must work collaboratively with family members and guardians to offer choices that support community integration. The provider does meaningful activities with people we support who reside in the community or residential. These services are provided in or outside the person we support’s home and are delivered on an individual basis. The services are designed to provide oversight and guidance to people supported so they can partake in their community to the fullest extent possible
Expectations:
- Maintain a positive work atmosphere by behaving and communicating in a manner that gets along with people we support, collaterals, and staff.
- Relate the Agency’s mission statement and core values to daily responsibilities, interactions, and decision-making.
- Adhere to the Agency’s Code of Conduct, Code of Ethics, Code of Professionalism, and Policies and Procedures.
- Use of the program’s software on their personal smartphone.
- Assume responsibility for the care and well-being of the people we support in the absence of their family member or caregiver.
- Act as a companion to the people we support(s), providing activities that are meaningful to the People we support.
- Carry out assigned duties as requested by the program supervisor and or the parent/caregiver.
- May transport individuals on trips into the community with a personal vehicle if required.
- Generally, 15-30 hours per week. Non-exempt position. Hours and days may vary based on the program's needs.
Responsibilities:
- Shall schedule service delivery times that meet the needs of the people we support and his/her family. Shall communicate with families about changes as they arise.
- Exercise flexibility in making up contact hours missed within the same week.
- Shall contemporaneously maintain accurate records regarding service delivery, utilizing the program software, such as dates, service delivery times, etc.
- Monitor, document, and submit in a timely fashion all expenses related to service delivery.
- Shall submit records to supervisor as per program policy.
- Shall maintain other records or summary notes as required.
- Responsible for monies allotted and records of the same.
- Develop and implement activities with their people supported. These activities need to be in accordance with the people’s needs and desires.
- Responsible for maintaining equipment/supplies as assigned.
- Perform other work-related duties as requested by the Coordinator of CSI.
REQUIREMENTS/SKILLS:
- Valid, clean driver’s license with one year driving experience required
- High School Diploma/GED preferred
- OMH and MR/DD experience is preferred
- One year of experience working with individuals with disabilities and knowledge and understanding of Community Hab policy required.
We generally work Monday thru Friday with flexible schedules between 8a - 6p . Based on the needs of the participants
