Long Term Care Specialist
Our growing medical centers are in search of an experienced Long Term Care Specialist
The ideal candidate will have had experience applying, following up, and ensuring compliance with the process of Long Term Care for patients in an Adult Activity/Day Care Setting.

JOB DESCRIPTION / JOB FUNCTIONS
· Call the Alliance to verify cases and to enter new patients
· Update patient records.
· Schedule PPDs for new patients and check that all current ones have it in good standing.
· Complete the patient's H&P. Ensure that all Adult Care Licenses are up to date and posted in a visible place.
· Complete forms 3008
· Control of the Chart and Files of Members, Employees and Subcontractors.
· Once these steps call the alliance
