Job title: After-Hours Telehealth Clinical Manager
Purpose: We are seeking a Nurse Practitioner or Physician Assistant to join our leadership team as a Clinical Manager for our After-Hours Telehealth program.
Role & Core Responsibilities:
- Reviews daily clinical performance metrics and implements interventions for areas of opportunity identified.
- Staff supervision leading and supporting the team by providing coaching, consistent feedback and ensuring adherence to workflows, policies and procedures.
- Performs hospitalization reviews to identify root cause and provides feedback to clients as part of the monthly clinical review meeting as well as the telehealth provider.
- Collaborates with Lumina Care team to recruit, hire, orient and onboard new clinical team members.
- Partners with Lumina Care Medical Directors, Collaborating Physicians, facility care teams and Lumina Care leadership to address clinical delivery of care concerns.
- Escalates any concerns with care delivery, relationships or any state/regulatory laws/regulations or employment law to the Director of Clinical Operations.
- Works closely with the Lumina Clinical Leadership team to ensure clinician quality and appropriateness of care is executed for all patients we serve.
- Identifies learning needs of team and works with Director of Clinical Operations and the Clinical Education team to address.
- Performs clinical audits through shadowing and documentation audits and provides feedback to clinicians. If further interventions are needed for performance improvement, partners with Director of Clinical Operations, the Clinical Education team, and Human Resources as appropriate.
- Conducts regular 1:1s with clinicians on team and provides feedback to applaud excellent work and implements a plan to address opportunities for improvement.
- Performs other related duties as assigned including the role and responsibilities as an After-hours Telehealth Clinician.
Supervisory Responsibilities:
- Will have a team of clinicians reporting directly to you for supervision, growth and development.
- Accomplishes department objectives by supervising staff and organizing and monitoring work processes.
- Nourishes clinical team growth by recruiting, orienting, and training clinicians and developing professional growth opportunities.
- Ensures quality by monitoring and maintaining clinical standards of care, compliance with health and safety regulations through audits and quality improvement initiatives.
- Accomplishes team results by coaching and performance management with assistance from the Director of Clinical Operations and HR.
- Develops and implements training in collaboration with the Clinical Education team.
- Implements and enforces workflows, policies, and procedures.
- Maintains a safe and healthy work environment by establishing and enforcing organization standards and adhering to legal regulations.
- Completes operations by developing schedules, assigning and monitoring work, gathering resources, implementing productivity standards, resolving operations problems, maintaining reference manuals, and implementing new procedures.
- Controls expenses by gathering and submitting budget information, scheduling expenditures, monitoring variances, and implementing corrective actions.
- Provides quality service by enforcing quality and customer service standards.
- Contributes to team effort by accomplishing related results as needed.
- Ensures compliance with all federal and state regulatory requirements in collaboration with the Clinical Leadership, Compliance, Legal and HR teams.
Credentials:
- Active and unrestricted state licensure in a minimum of five states and willingness to obtain additional if requested.
- Active and unrestricted Advanced Practice Nurse, National Board certification (AANP, ANCC or AACN) OR active and unrestricted Physician’s Assistant NCCPA board certification.
- Active DEA licensure, or willingness to obtain in the states of practice is required upon hire.
- Must have an active national provider number (NPI).
- Must have active Medicare & Medicaid provider numbers, or willingness to obtain them upon hire.
- Must be eligible to participate in the Medicare & Medicaid programs as a provider.
Qualifications:
- A minimum of five years' experience as a Nurse Practitioner or Physician Assistant is required.
- A minimum of one year's experience in a leadership role with direct management of employees is preferred.
- Highly organized and able to manage multiple projects, people, and deadlines at one time.
- Is excited to become a member of the clinical leadership team to coach and grow team members promoting clinical excellence.
- Long term care experience is preferred
- Telehealth experience is required
- Experience managing patients with polypharmacy, completing gradual dose reductions (GDRs) and addressing deprescribing opportunities is required.
- Technology proficiency required.
- Excellent people skills: communicating with patients, families, and members of the healthcare team.
- Self-directed with the ability to practice autonomously, but functions as a member of the team.
- Excellent clinical skills with an ability to assess, diagnose, develop a treatment plan and communicate in a virtual environment.
Working conditions:
A quiet work from home space and a functional computer are required for this remote role. Typing is required, as well as using virtual technology to visualize patients during visits. Periods of prolonged sitting or standing can be expected. The ability to work weekends, nights, and holidays is required. Travel requirements could occur based on specific state licensure and leadership needs.
Compensation:
This role will be full-time and will be composed of clinical leadership work (daytime hours) as well as after-hours tele-health work. This division of time will be fluid based on company needs and initiatives.
$60-$75 per hour (within the range, geography, individual pay is determined by job-related skills, experience, licensure, and relevant education or training).
