The MDS Coordinator manages the resident assessment process and coordinates clinical documentation across departments. You will work directly with nursing staff and department leaders to set assessment reference dates and verify care plan accuracy. Your daily focus is ensuring baseline assessments and routine MDS section coding match resident medical records.
Responsibilities
- Complete MDS assessments and care plans according to federal schedule timelines.
- Coordinate assessment reference dates with nursing leadership and therapy teams.
- Review resident charts and MARs/TARs to ensure documentation accuracy.
- Transmit completed MDS data to state databases within mandated timeframes.
- Participate in weekly care planning meetings with facility leadership and resident families.
Compensation, Schedule & Benefits
- Pay Rate: Starting at 35/hr, increases with experience
- Schedule: Monday–Friday (8am-5pm)

Qualifications & Requirements
- Active Texas RN or LVN license in good standing
- 1–2 years of experience with MDS 3.0 and RAI processes
- Must pass a standard criminal background check
- Must pass a drug screening
- Reliable transportation for dependable attendance
