Purpose:
The Transportation CNA is responsible for transporting residents of the healthcare center to and from appointments & companionship. The Transportation CNA is responsible for ensuring that service is delivered in a caring and respectful manner, in accordance with applicable federal, state, local and facility standards, guidelines and regulations and as may be directed by the Director of Nursing and/or The Nursing Home Administrator. The goal of this position is to transport healthcare residents to appointments and organize the scheduling of those appointments.
Reports To: Director of Nursing
Primary Job Duties:
· Escort residents to medical appointments and outings as specified in the care plan.
- Participate on the Care Team by providing input and making suggestions.
- Follow the written care plan.
- Coordinate the scheduling of all follow up medical appointments for the Healthcare Residents.
- Assist with podiatrist, ophthalmology, and dental appointments when provided on site at Laurel View Village.
- Carry out duties as assigned by the Director of Nursing
- Attend orientation, in-service training sessions, drivers certification and staff meetings according to facility guidelines.
- Develop and maintain constructive and cooperative working relationships with others.
- Make decisions and solve problems.
- Communicate with Director of Nursing and co-workers.
- Observe, receive, and obtain information from relevant sources.
- Performs other duties as required.
- Inputting all transportation information into the WorxHub system.
- Scheduling vehicle in coordination with other departments including: Life Enrichment, Home Care, Transportation and Healthcare
- Completes annual compliance and HIPAA training and exhibits behavior as set forth by the Code of Compliance in the performance of their duties.
- Adheres to the Organizations Policies and Procedures regarding the Compliance Program of the organization and is timely in reporting any possible concerns.
- Follows all local, state, and federal regulations as they pertain to their position.
- Adheres to the Organization’s Policies and Procedures covering Privacy and Security in Compliance with HIPAA regulations.
- Coordinates and obtains authorizations from insurance companies for transportation and procedures as needed.
Required Knowledge:
- Knowledge of the English language.
- Knowledge of the information and techniques needed to diagnose and treat injuries including emergency first aid and CPR.
- Knowledge of clerical procedures such as maintaining records and completing forms.
- Knowledge of medical transportation processes, roles, and responsibilities.
Working Conditions:
· May be exposed to infectious waste, including COVID 19, HBV and HIV.
· Sits, bends, stands, lifts, and moves throughout the facility during the workday.
· May be required to work on weekends, holidays, and on shifts other than the one normally scheduled.
Other Requirements:
- Must have a valid Certified Nurse Aide Certification in the state of Pennsylvania
· Must be able to deal tactfully with supervisors, co-workers, residents, family members, visitors, government agency staff and the public.
· Must be able to move throughout the facility during the workday.
· May be required to lift, push, pull, and move equipment and supplies more than (50) fifty pounds during the workday.
· Must maintain confidentiality of resident information.
· Must be able to work independently and follow instructions given by supervisor.
· Must attend required training and education sessions offered by the facility.
Education and Experience Requirements:
· Must have a high school diploma/GED equivalent
· Must be a valid, current certified nursing assistant.
· Must have a valid driver’s license
· Will be required to obtain and maintain CPR certification
